Please include the following information:

1. Name;
2. Address;
3. Social Security number -- or --
   KTRS member identification number
4. Current year salary;
5. Beneficiary's birth date;
6. Projected retirement date;
7. If the employer pays for sick days, provide balance of unused sick leave; -- and --
8. Projected salary increases for each year beyond the current year.
